Current Situation & Your Options
Due to very high demand, the NHS ADHD service in Derbyshire is temporarily closed. The good news is that you can still access an assessment through a process called Right to Choose. This allows you to choose a private provider, and the NHS will fund the assessment.

Choosing a Provider
Before making your decision, it’s important to check the following:
- You meet the provider’s referral criteria
- They are able to prescribe medication (if that’s something you may want)
- You’re aware of their waiting times
We understand you may want to be seen quickly, but it’s essential to choose the provider that best suits your needs – not just the one with the shortest wait.
Next Steps – How to Request a Referral
To start the referral process, please complete all five of the documents listed below and return them to us:
- ADHD Screening Questionnaire
- A written or typed statement detailing:
- Your symptoms and concerns – if you have not had these since childhood, your referral is not likely to be accepted
- How ADHD affects your everyday life (home, work, relationships)
- What you’ve tried to manage these symptoms
- What you hope to gain from a diagnosis
(Please provide as much detail as possible to help your referral succeed.)
- The name of the Right to Choose provider you’ve selected
- The referral proforma downloaded from the provider’s website(Note: Your GP cannot complete this form for you.)
- The ADHD Right to Choose Disclaimer Document, signed by you

Please return the documents to us by bringing them to the surgery. Alternatively you can post/email these to us.
We’ll begin your referral only when we receive all five items. Incomplete submissions may cause delays or be rejected.
Reminders & Timeframes
If any documents are missing, we’ll send you reminders. We’ll keep your referral open for 21 days to give you time to send everything. If we don’t hear back, we’ll assume you no longer wish to continue and will close your referral – this helps our admin team support other patients too.
No Need to Speak to a GP First
There’s no requirement to speak with a GP before submitting your documents. We’ll review everything you send in to assess eligibility.
After You’re Referred
Once your referral is sent to your chosen provider, you’ll hear directly from them. From that point on, any questions or updates should go directly to your provider – our surgery will no longer be involved in the process.
