Going home from Hospital Check list

We understand that after a hospital stay you may be tired, unwell, or understandably eager to get home, and that waiting for discharge medications can sometimes be frustrating. However, it is important that the care recommended by the hospital team is completed before you leave.

Prescribing is not an administrative task. It is a clinical decision that carries responsibility, risk and therefore time. The hospital team that has assessed you, made the diagnosis and recommended the treatment is responsible for prescribing it.

The same applies to fit notes (sick notes). If you need one following your admission or treatment, please ask the hospital team before discharge.

We work hard to ensure good access for you, for the work we are funded for. Sadly we cannot also take on work that should have been completed by the hospital, particularly when we have not yet received details of your admission and treatment.

Before leaving hospital, please check that you have:

• All medications you need (14 days provided)

 • Any promised prescriptions

 • A fit note if required

 • Clear follow-up instructions

Taking a few moments to check these things before you leave can help avoid delays, additional appointments, and unnecessary stress once you are home.
